WORKSHOP REGISTRATION OPTIONS AND RATES
Please select all that apply from the options below. Be sure to double-check the rate that you select.
IPOS or SEPO Members, Nurses from Spain: Only IPOS or SEPO members who have paid their 2008 membership dues, or nurses residing in Spain, are eligible for the member/nurse rate.
Students: Student rates may apply to students and trainees, including residents and fellows. To qualify for this rate, your advisor/department chair must sign and date a letter on department letterhead confirming your status, and the letter must be mailed to IPOS, 2365 Hunters Way, Charlottesville, VA 22911, USA, faxed to +1 434.977.1856, or scanned and e-mailed to info@ipos-society.org.
LMI Country Residents: Low- or Middle-Income (LMI) country residents may register at reduced cost. Country income status is determined by the World Bank. Please see the LMI Country List to determine if you qualify for this reduced fee status.
PLEASE DO NOT USE QUANTITIES GREATER THAN 1 for Workshop Registration Options C or D. A separate order must be placed for each attendee, so we may collect each attendee's name and contact information.
