Global Authorities Declare Distress the 6th Vital Sign in Cancer Care
IPOS Statement on International Standards for Quality Cancer CareDistress may soon be universally ranked as the 6th vital sign in cancer care, following the Union of International Cancer Control’s (UICC) endorsement of a recommendation that cancer care standards and clinical practice guidelines be revised. The recommendation was made by the International Psycho-Oncology Society (IPOS).

President of the UICC, Dr Eduardo Cazap, said the endorsement would improve cancer care guidelines and practice internationally. “The UICC is in unanimous agreement with IPOS that distress must be a primary concern in the treatment of cancer patients,” he said. “The high prevalence of distress in cancer patients world-wide has been well documented, with significant impacts on patients’ basic functioning. We expect that recognising distress as the 6th vital sign will improve the treatment of cancer patients, improve outcomes for cancer patients, and improve the effectiveness of cancer care systems around the world,” Dr Cazap said.

President of IPOS, Professor Maggie Watson, said UICC endorsement paved the way for World Health Organisation ratification of the recommendation. “WHO endorsement would be welcomed by cancer patients, survivors, and supporters world-wide. Importantly, the UICC’s endorsement provides a mandate for member organisations to call on their home nations to rank distress as the 6th vital sign in cancer care,” she said. “This will inspire historic reform of standards and guidelines for clinical practice, with positive implications for international cancer control. The effect of distress on cancer patients, families and the community has largely been overlooked in models of primary care, despite the serious consequences of distress on patients’ prospects of long-term survival and quality of life,” Prof Watson said.

In routine medical practice vital signs assess basic functioning, and include body temperature, pulse (heart rate), blood pressure, respiratory rate, and pain. The IPOS recommendation was officially endorsed by the UICC at its board meeting prior to the 2010 World Cancer Congress in China.

IPOS Endorses Distress as the 6th Vital Sign in Oncology
At the IPOS 2009 Board Meeting in Vienna, members unanimously endorsed the concept that Distress be named the 6th Vital Sign in Oncology. In routine medial practice Vital Signs are utilized by health professionals in order to assess basic functioning. Vital signs are an essential part of patient’s case presentation and generally include: Body Temperature, Pulse (or heart rate), Blood pressure, and Respiratory rate. In 1999, due to the significant impact on patients, Pain was endorsed as the 5th Vital Sign.

Prevalence rates of Distress in cancer patient populations have been well documented in the literature. The impact of distress on cancer patients, families and the community is a primary concern for clinicians, scientists, educators, and IPOS.

Distress as the 6th Vital Sign was first endorsed by the Canadian Strategy for Cancer Control in 2004. The concept of naming Distress as the 6th Vital Sign has gained much support and has been published in Journal of Clinical Oncology, the Journal of Psycho-Oncology, and the Journal of the National Comprehensive Cancer Network to name but a few.

IPOS believes that endorsing Distress, as the 6th Vital Sign will raise awareness of Distress as an essential component of the patient experience and will act as a call to action for care providers and the health care system as a whole.

Access to Pain Treatment as a Human Right (from the Human Rights Watch)
The Human Rights Watch has published a 47-page report entitled, "Please, do not make us suffer any more..." Access to Pain Treatment as a Human Right. The report states that countries could significantly improve access to pain medications by addressing the causes of their poor availability. These often include the failure to put in place functioning supply and distribution systems; absence of government policies to ensure their availability; insufficient instruction for healthcare workers; excessively strict drug-control regulations; and fear of legal sanctions among healthcare workers. The report's website hosts PDF versions of the report and recommendations in English, French, Russian and Spanish.

IPOS Gives Extra Congress Bags to African Students
After the IPOS 9th World Congress of Psycho-Oncology in 2007 in London, UK, there were approximately seventy-five Congress bags that had not been used by delegates. Janis Homewood, a colleague of Congress Chair Maggie Watson, was planning a trip to Africa with the Salvation Army and volunteered to carry the bags for distribution at a local school. The students were very excited about their new bags, and we hope that some of the students will pursue careers to bring them to future IPOS Congresses!
